THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

PT2U LLC is committed to obtaining, maintaining, using and disclosing patient protected health information in a manner that protects patient privacy. We urge you to read this Notice of Privacy Practices (“Notice”) carefully in order to understand both our commitment to the privacy of your protected health information and your rights.

PT2U LLC is required by law to maintain the privacy of your protected health information and to provide you with a notice of our legal duties and privacy practices with respect to protected health information. This Notice describes how we may use and disclose your protected health information to carry out treatment, payment or health care operations and for other specified purposes that are permitted or required by law. This Notice also describes your rights with respect to your protected health information. “Protected health information” or “PHI” is information about you, including basic demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.

PT2U LLC is required to follow the terms of this Notice. We will not use or disclose your PHI without your permission, except as described in this Notice. We reserve the right to change our practices and this Notice as and to the extent permitted by law and to make a new Notice effective for all PHI we maintain. Any new Notice will be available upon your request and will be posted on our website.

Your Rights

  • OBTAIN A COPY OF YOUR MEDICAL RECORD: You always have the right to ask for your medical records and we will supply them within a reasonable legal timeframe, usually within 30 days of your written request. For bulk requests we reserve the right to charge a reasonable cost-based fee.
  • ASK FOR A CORRECTION TO YOUR MEDICAL RECORD: If you think there is incorrect information we will correct any errors within 60 days. If we do not deem there to be an error we will respond in writing within that same time period.
  • REQUEST A COMMUNICATION CHANGE: You can ask for a change in how we communicate with you, and we will agree to all reasonable requests that also allow us to maintain our legal compliance with communicating health and treatment items.
  • CHOOSE SOMEONE TO ACT FOR YOU: If you have a legal guardian or have given someone medical power of attorney we will allow that person to make choices about your health information once we have verified that person's authority.
  • ASK FOR A LIST OF THOSE WHOM WE'VE SHARED YOUR INFORMATION: We will supply this in all cases except where a law prohibits it. We will provide this annually for free, but subsequent requests may incur a reasonable labor charge.
  • ASK US TO LIMIT SHARING YOUR INFORMATION: You can ask us to not share specific information for payment, treatment, or clinic operations. We will agree to all requests within our legal ability, but there are some mandatory reporting requirements we are held to. We will answer any questions or concerns in this regard.
  • OBTAIN A COPY OF THIS PRIVACY NOTICE: You can ask for a paper copy of this notice at any time and we will provide it promptly.
  • FILE A COMPLAINT IF YOU FEEL YOUR RIGHTS ARE VIOLATED: You can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Ave., S.W., Washington. D.C., 20201, or by calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints.

How do we use or share your protected health information

  • Treating you by sharing your health information with other professionals who are also currently treating you.
  • Complying with record requests for other offices who have verified they are actively treating you.
  • Using your information to run our practice, improve your care, and communicate with you as necessary.
  • Bill for your services with health plans and other entities, utilizing the least information necessary approach.

Privacy Policy

At PT2U LLC we are committed to your privacy, and maintain specific company guidelines:

  • We only ask for personal information that we truly need, or are required by law to obtain.
  • We only share your information with verified, authorized, entities, and only after they have demonstrated appropriate right and need to that information.
  • We only store information on your servers as is required for the continued operation of our site, and within our protected electronic medical record system.

Website Visitors
Like most website operators, PT2U LLC collects non-personally-identifying information of the sort that web browsers and servers typically make available, such as the browser type, language preference, referring site, and the date and time of each visitor request. PT2U LLC's purpose in collecting non-personally identifying information is to better understand how its visitors use its website. From time to time, we may release non-personally-identifying information in the aggregate, e.g., by publishing a report on trends in the usage of its website.

Certain visitors to PT2U LLC’s website choose to interact with PT2U LLC in ways that require us to gather personally-identifying information. The amount and type of information that PT2U LLC gathers depends on the nature of the interaction. PT2U LLC does not disclose personally-identifying information other than as described below. For instance, PT2U LLC may monitor the most popular pages on the list25.com site or use spam screened by the Akismet service to help identify spam. We primarily use our various product blogs to communicate this type of information, so we expect to keep this type of email to a minimum. If you send us a request (for example via a support email or via one of our feedback mechanisms), we reserve the right to publish it in order to help us clarify or respond to your request or to help us support other users. PT2U LLC takes all measures reasonably necessary to protect against the unauthorized access, use, alteration or destruction of potentially personally-identifying and personally-identifying information.

Cookies:
A cookie is a string of information that a website stores on a visitor’s computer, and that the visitor’s browser provides to the website each time the visitor returns. Authentic Hand Therapy uses cookies to help Authentic Hand Therapy identify and track visitors, their usage of Authentic Hand Therapy website, and their website access preferences. Authentic Hand Therapy visitors who do not wish to have cookies placed on their computers should set their browsers to refuse cookies before using Authentic Hand Therapy’s websites, with the drawback that certain features of Authentic Hand Therapy’s websites may not function properly without the aid of cookies.

Business Transfers:
If PT2U LLC, or substantially all of its assets were acquired, or in the unlikely event that PT2U LLC goes out of business or enters bankruptcy, user information would be one of the assets that is transferred or acquired by a third party. You acknowledge that such transfers may occur, and that any acquirer of PT2U LLC may continue to use your personal information as set forth in this policy.

Ads:
Ads appearing on any of our websites may be delivered to users by advertising partners, who may set cookies. These cookies allow the ad server to recognize your computer each time they send you an online advertisement to compile information about you or others who use your computer. This information allows ad networks to, among other things, deliver targeted advertisements that they believe will be of most interest to you. This Privacy Policy covers the use of cookies by PT2U LLC and does not cover the use of cookies by any advertisers.

SMS Messaging:
SMS opt-in or phone numbers for the purpose of SMS are not being shared.

Comments:
Comments and other content submitted to anti-spam services are not saved on our servers unless they were marked as false positives, in which case we store them long enough to use them to improve the service to avoid future false positives.

Privacy Policy Changes:
Although most changes are likely to be minor, PT2U LLC may change its Privacy Policy from time to time, and in PT2U LLC’s sole discretion. PT2U LLC encourages visitors to frequently check this page for any changes to its Privacy Policy. Your continued use of this site after any change in this Privacy Policy will constitute your acceptance of such change.
